Glenda, I wanted to tell you thank you. Last year I printed some you had posted. I filled them out and put one in my Mom's car (75 years young), my Daughter's car, mine & my Sister-In-Law's car. My Mom was in an accident. I was notified immediately ! Thanks to the ICE form the Police Officer found in her car. At the hospital the officer told me he wished EVERYONE carried this info.
